Docket® - Immunization Records 2.2.70

Docket® partners with public health departments in Alaska, Idaho, Minnesota, New Jersey, Utah, and Wyoming to provide users with easy access to personal and family immunization records. By connecting directly with these state health departments, Docket® allows you to efficiently track upcoming shots, review past immunizations and securely share official immunization reports.

Residents of Alaska, Idaho, New Jersey, Utah, and Wyoming can take advantage of the SMART® Health Card vaccine credential QR code offered by Docket®, which can facilitate travel to locations with COVID-19 restrictions. Keep an eye out for updates on additional states joining this service.
